We provide care that addresses our residents' physical, spiritual and emotional needs. Our staff consists of a group of trained professionals dedicated to meeting the needs of each resident. We work as a team-from laundry and maintenance, to dietary and nursing, to administrative staff-and take pride in providing a friendly and caring atmosphere.
We offer a comprehensive activity program, with a wide variety of options, from exercise programs to parties. You can structure your day to suit your preferences-busy or relaxed!
We understand the importance of variety in meals and offer an a la carte dining program that allows residents to order from a menu, choosing from an array of delicious entrees and side dishes.
The Plaza at Mansfield has several payment options, including Part A and B of Medicare, private insurance, Medicaid, respite care programs, participating HMOs and private pay.

Our skilled nursing residents benefit from a wide array of services:
* 24-hour specialized nursing care
* Physical and occupational therapy
* Speech/language pathology
* Respiratory therapy
* Laboratory, x-ray, and other diagnostic services
* Nutritional management
* Social services and case management
* Recreation program
Each skilled nursing resident will receive an individualized treatment plan designed for optimum recovery. Goals are set for each resident by an interdisciplinary team comprised of the resident and/or family members, the participant's physician and our health-care professionals. Weekly team meetings are held to review the progress of the resident and re-evaluate the treatment plan.
Once the goals have been achieved or the team determines the resident is no longer progressing through the treatment program, he/she will be discharged home or to an environment that will best meet their needs.
