Senior Care Health & Rehabilitation is the newest nursing facility in Texoma. Our western- ranch décor provides elegant common areas, dayrooms, and dining. Our spacious resident rooms are equipped with flat screen televisions & private bathrooms. We have secure outdoor spaces to enjoy the beautiful landscaped grounds and gazebos. The therapy and skilled nursing unit provides Private and Semi Private Medicare rooms each with in-room private phone lines at no additional cost.
We specialize in short-term skilled nursing therapy services. Since opening in October 2007, our therapy and nursing teams have discharged more than twice the national average home or to prior living situations. We are extremely proud to say 68% of residents admitted to SCHR return home after receiving therapy services.
Working to meet therapy needs
Our in-house Rehabilitation Team works closely with the nursing staff to ensure an interdisciplinary approach to our rehabilitation programs.
Our licensed therapy staff specializes in treating the geriatric client including:
Post Surgical rehabilitation, Orthopedic rehabilitation, Post-stroke rehabilitation, & Neurological rehabilitation. We offer Wii Rehab as well as the Accelerated Care Plus (ACP), which is state- of-the-art therapeutic modalities with evidence based protocols. ACP includes Diathermy, Ultrasound, PENS (Patterned Electrical Neuromuscular Stimulation) & Electrical Stimulation which allows us to treat more complex conditions and accelerate recovery in patients with both orthopedic and neurological diagnosis. The specialized treatment protocols include physical agent modalities for COPD, Rheumatoid Arthritis, Pain Management, Contracture Management, Fall Prevention, Bladder Diagnostics & Continence Improvement, Wound Healing, Stroke Recovery, & Edema Management.

The professional licensed nursing staff is experienced in IV therapy, Wound Care including Wound Vac therapy, Tracheotomy care, Diabetes management, & Palliative care including Pain Management. The Medical Directors, Raymond Owen M.D. & Jorge Valencia, M.D., oversee the continuity of care for those needing skilled nursing and therapy services.
