Welcome to the Gilmer Care Center Website. We are a Long-term care facility located in Gilmer, Texas which is located in the beautiful Northeast area of Texas. Our facility has 112 bed capacity that services a large variety of needs. We have a 22 bed Skilled Medicare wing as well as a 20 bed Alzheimer's unit (The Garden) Certified by the Texas Department of Human Services.

Gilmer Care Center provides a variaty of services and accepts several pay sources. Our staff is highly qualified and we are continuously working to exceed all State and Federal standards.

Services:

Gilmer Care Center is dedicated to the physical, mental and spiritual well being of each resident. We offer several services to keep our residents active. We have scheduled activities each day of the week both in groups and as individual activities. We provide in-house therapy making it convenient for our residents to receive therapy without leaving the building. Gilmer Care Center also offers a 20 bed Alzheimer's Unit "The Gardens".

Rehabilitation:

At Gilmer Care Center our therapist team works toward the goal of returning the patient home or to the highest functional level of life, which might require Physical, Occupational, Speech or Respiratory therapy. Gilmer Care Center has an in-house therapy department that takes a hands on approach which begins with orders from a physician, examination, development of treatment program specifically designed to meet the needs of the patient. We provide both in-patient and out patient Therapies. Weekly performance evaluations are conducted while on therapy to monitor the progress of the patient, family and patient education and training are conducted during the therapy experience and upon discharge a home exercise program or nursing restorative programs is setup for the patient.

Gilmer Care Center's Therapy room consists of twelve hundred square foot facility with therapeutic equipment including; stairs, parallel bars and a "mock" house setup designed to prepare the patient for a safe transition back to home. Our team welcomes family participation, questions and concerns about the plan of care.

Physical Therapy: provides skilled therapy to improve the patient's ambulation, mobility, balance, strength, transfer ability and range of motion. Therapists assess the need for specialized braces or equipment that may be required to improve functional performance and safety and provide pain reduction. Treatment includes neuromuscular facilitation therapeutic exercise, gait analysis, joint protection techniques and fall prevention, energy conservation, therapeutic modalities, and patient education.

Occupational Therapy: helps the patient retain self-care skills such as bathing, grooming, dressing and feeding. Occupational therapist are sensitive to the stress and frustration often felt by the patient and family members during the rehabilitation period. Occupational therapist specialize in preparing the patient for environmental changes and assisting the patient in making the changes in a safe manor. Treatment includes functional mobility, self-care activities, positioning techniques, contracture management, home safety assessment and activity analysis.

Speech Therapy: Speech-language pathologists assist patients with improving oral-motor skills, swallowing, communication, cognition, and voice production. Patients whose comprehension and interactive skills are reduced due to age related hearing impairment or other medical impairments are often referred to a speech language pathologist. Treatment includes teaching techniques to improve swallowing, feeding, enhance communications, listening, reading and writing comprehension, language and expressive skills.
