Countryside Nursing & Rehabilitation is licensed to serve the needs of 120 residents. The facility is certified to provide both Medicare and Medicaid services.

The facility also has an active resident council.
An interdisciplinary care plan team will work hand-in-hand with each resident's physician to develop a plan of care for your loved one. Their purpose is to create a plan of care which is tailored to each resident's individual needs.
The interdisciplinary care planning team, which consists of nursing, social services, activities, dietary, and rehabilitation meet at least quarterly to review each resident’s progress and determine the proper course of action or “plan of careâ€. We strongly encourage families to attend and participate in this care planning process, as your experience and knowledge of your loved one's interests and activities can greatly improve this process.
