Azalea Trail Nursing and Rehabilitation Center is a "God, family and friends operated" long term care facility. Diane Ward, along with her husband Michael, work in the day-to-day operation of the company. Four of their five sons, a sister and daughter-in-law work with them, as well as numerous friends. Many of their business contacts are personal friends and have the same vision for relationship in business.
"Our Mission Statement", as Diane Ward says, "is our hope that these people will be the essence of relationships that extend to the staff and residents who will want to work and live in our facility."
Nursing:
Azalea Trail Nursing and Rehabilitation Center has a multidisciplinary approach to meeting the needs of residents. This consists of an interdisciplinary team which includes a staff of Licensed Nurses, both Vocational and Registered, Certified Nurses Aides, Therapist (Physical, Occupational & Speech), Social Worker, Activities Director, Treatment Nurse, and Medical Director.
Regular assessments are done to determine the needs of each individual resident. These needs are developed into a Plan of Care during regularly scheduled ITTM (Interdisciplinary Team) Meetings. This Plan of Care is formed by the input of the multidisciplinary team, the resident's physician, the resident and family members. The nursing team is lead by a Director of Nursing and an Assistant Director of Nursing, both with long term care experience. It is their responsibility to oversee the rest of the nursing staff and ensure the Plan of Care is carried out.
The nursing team is available to provide assistance to the resident as needed with activities of daily living, including hygiene, grooming and dressing, and assistance at meal times. A licensed nurse or certified aid administers medications as well as other interventions or treatments ordered by the physician including respiratory therapy and wound care as prescribed.
Rehab:
Azalea Trail Nursing and Rehabilitation Center provides on-site Rehabilitation to individuals whose level of independence is threatened by physical injury or illness. Assessment and training in functional skills by licensed Physical, Occupational and Speech Therapists helps the residents to achieve more closely their former abilities. With Restorative Nursing, in addition to patient and caregiver education, residents of Pyramid HealthCare Facilities are able to continue to improve even when skilled therapy is no longer needed.
Stroke, Parkinson, Joint Replacement Surgeries and Amputations are just a few of the diagnosis the Rehab Teams are experienced in treating. With an array of clinical exposure prior to long term care, the therapists can address all of their residents' rehab needs. Therapy is also provided to those who need it as an extension to hospitalization, to maximize safety and independence before returning home.
