24-hour skilled nursing care and effective rehabilitation programs offered in an environment of respect and concern for the whole person.

Skilled Nursing Services:

Led by our Director of Nurses, experienced Registered Nurses, Licensed Vocational Nurses, Medication Aides and Certified Nurses Aides provide our residents with nursing care. These activities include assisting residents with their activities of daily living, promoting and restoring each resident’s health when possible, preventing illnesses and evaluating each resident’s on-going medical condition. We coordinate with each resident, their family and their physician in establishing goals for their care.

The staff has the capability to meet the medical needs of those residents who require very complex treatments and therapies. We work closely with each resident’s attending physician so that instructions for treatments, medications, therapies and dietary requirements are followed precisely.

Our Interdisciplinary Care Team, consisting of a Registered Nurse, Social Worker, Dietitian, Activities Specialist, Physical, Speech and Occupational Therapists, develop an individualized plan of care for each resident. This team meets regularly to review and update the resident’s plan of care. The resident, the family and the resident’s physician are all invited to participate in this process. We are continually seeking new ways to enhance each resident’s quality of life.

Our nursing department believes each person is entitled to the highest level of wellness possible and is dedicated toward striving for that result.

Dining and Nutrition:

We recognize the importance of mealtime to our residents and appreciate the benefits of good nutrition in adding to one's well being. Each resident's diet, whether regular or modified, is ordered by their attending physician and monitored by our staff, thus helping to maintain one's nutritional health. A registered dietitian continually examines the nutritional value of the meals and snacks offered to our residents.

Dining & Nutrition at Prestonwood RehabPrestonwood Rehabilitation and Nursing Center has two dining rooms in order to accommodate the needs and preferences of our residents. The socializing that occurs during dining contributes to the resident's enjoyment of their meals.

Our dietary staff will gladly honor special requests for birthday gatherings or anniversaries. A resident's family and friends are welcome to dine with their loved one. A call in advance will assist us in making the necessary arrangements.

Rehabilitation Services:

Our Therapy Team specializes in assisting those residents who qualify for services aimed at regaining as much of their independence as possible. This team includes Physical, Occupational and Speech Therapists. The goal of this team of professionals is to facilitate the optimal level of functionality for the residents; in many cases this may lead to returning home.

Physical Therapy
Physical therapists provide skilled therapy to patients to improve their ambulation, mobility, balance, strength, transfer ability and range of motion. Physical TherapyThey instruct patients in skilled techniques to improve their ability to walk, climb stairs and increase physical stamina. Physical therapists assess the need for specialized braces or equipment that may be required to improve functional performance, self-reliance and safety. They also provide skilled therapy for pain reduction and pain management techniques. Treatment includes neurovascular facilitation, therapeutic exercise, gait analysis, joint protection techniques and conservation, fall prevention, therapeutic modalities and patient education.

Occupational Therapy
Occupational therapists retrain patients in self-care skills such as bathing, grooming, dressing and eating. They use graded activities, safety training and compensatory techniques to improve activities of daily living. Occupational therapists are specialists in recommending adaptive equipment and environmental changes to help residents function safely in their surroundings. They are especially sensitive to the stress and frustration often felt by patients and their families during the rehabilitative period. Treatment techniques include functional mobility, self-care activities, positioning techniques, contracture management, home safety assessment and activity analysis.

Speech-Language Therapy
Speech-language therapists assist patients of all ages with improving oral-motor skills, swallowing, communication, cognition and voice production. They provide skilled intervention for residents who demonstrate language and comprehension deficits secondary to illness, injury or developmental delay. Additionally, patients whose comprehension and interactive skills are reduced due to cognitive loss or hearing impairment are often referred to speech language therapists for recommendations of augmentative or alternative communication systems or for patient and caregivers training to avoid patient isolation. Treatment strategies involve teaching compensatory techniques to improve swallowing and feeding and to enhance communication, listening, reading and writing comprehension, language interpretation and expressive skills.

In addition to the therapy services listed above, the Therapy and Nursing Departments supervise the work of our team of restorative aides who assist our residents in maintaining their physical strength and independence to as great a degree as possible.
