We provide state-of-the-art rehabilitation, with short- and long-term nursing care options. A full-time activity director works with residents daily, providing fun and healthy activities with lots of variety. An on-site chaplain provides spiritual care to residents and their families.

We understand that delicious meals are important to our residents' overall quality of life. We have designed our dining room with comfortable, yet elegant touches to make mealtime one of the most pleasurable times of day. In addition, we offer an enhanced dining program that allows residents to order from a menu, choosing from an array of delicious entrees and side dishes.

Green Oaks has several payment options, including Part A and B of Medicare, private insurance, Medicaid, respite care programs, participating HMOs and private pay.

Our skilled nursing residents benefit from a wide array of services:

* 24-hour specialized nursing care

Each skilled nursing resident will receive an individualized treatment plan designed for optimum recovery. Goals are set for each resident by an interdisciplinary team comprised of the resident and/or family members, the participant's physician and our health-care professionals. Weekly team meetings are held to review the progress of the resident and re-evaluate the treatment plan.

Once the goals have been achieved or the team determines the resident is no longer progressing through the treatment program, he/she will be discharged home or to an environment that will best meet their needs.
